DATA SUBJECT APPLICATION FORM FOR APPLICATIONS TO BE MADE BY THE PERSONAL DATA OWNERS TO THE DATA CONTROLLER
General Statements:
In the Personal Data Protection Law No. 6698 ("Law"), it is regulated that the personal data subjects defined as the person concerned ("Data Subject") may use the rights granted to their personal data in Article 11 of the Law and must contact the Data Controller to exercise these rights.
In accordance with paragraph 1 of Article 13 of the Law; Applications to KELESOGLU HOLDING INC ("Data Controller" or "Company") as data controllers must be submitted to us in writing.
Application Procedure:
In this context, the applications to be made to our company "in writing";
You can make your request with one of the following methods, which includes the necessary information to identify you and your explanations about your right to use the rights specified in Article 11 of the Law No. 6698:
- 1. After filling out the form at the application form http://www.kelesogluholding.com a copy of the wet signed document is delivered to “Marmara Mh. Ulusum Cd Outer Door No:4/39 Inner Door No:10 - İstanbul Beylikdüzü/İst/ Turkey” in accordance with the Law, by hand or through a notary,
- 2. After filling out the form at http://www.kelesogluholding.com address and signing it with your "safe electronic signature" under the Electronic Signature Law No. 5070, the safe electronic signed form is sent to the [email protected] or by e-mail registered to the [email protected] address.
The aforementioned are the "written" application places in accordance with paragraph 1 of Article 13 of the Law. It will be announced by our company how the applications will be received through these methods after the other methods to be determined by the Board are announced.
Your applications submitted to us will be answered free of charge "as soon as possible and within thirty days at the latest" from the date, your request reaches us according to the nature of the request, in accordance with paragraph 2 of Article 13 of the Law. However, if the transaction also requires a cost, the fee will be charged by our company at the receipt determined by the Personal Data Protection Board. Our responses will be delivered to you in writing or electronically in accordance with article 13 of the relevant Law.
According to the nature of this form and your request, the information and documents requested from you must be provided to us completely and accurately. If the requested information and documents are not provided properly, there may be disruptions in the full and qualified execution of the researches to be carried out by our company at your request. In this case, our company declares that it reserves its legal rights. Therefore, the relevant form must be submitted in complete and requested information and documents according to the nature of your request.
